On Mobile:
From your Instagram Account, tap the three horizontal lines at the top right corner to go to Settings and Activity.

On Instagram Web:
Click the three horizontal lines at the bottom left then tap Your Activity.
Click Account History.
The downside when using Instagram Web is not being able to filter by type. It will only allow you to filter by date.
How To Check Other Accounts' Old Instagram Usernames
Checking another Instagram Account's former usernames is impossible. However, you can view the number of times an Instagram Account has changed its username under Former Usernames.
Go to another Instagram user's account.
Tap the three dot menu icon at the top right corner then click About this account.
If you do not see former usernames on their About this Account section, it means that they never did any username changes.
Tip: The closest thing you can do if you want to check someone else's previous username is to scroll through their posts and check the comments section.
